There are important health requirements that are needed as part of school enrollment, and throughout the student’s scholastic career, which are based upon New York State’s Educational Laws. Please review the information below and contact us if you have any questions.
Health Examinations (physicals):
- Required for all new entrants, regardless of grade, and students in grades Pre-K or K, 1, 3, 5, 7, 9 & 11;
- Every year for students participating in athletics (sports);
- Every year for working papers as needed; or
- When required by the Committee on Special Education (CSE) or Committee on Preschool Special Education (CPSE).
*A dental exam is also requested at the same time a grade-level health examination is required. Please note, this is not a requirement for school attendance.
Immunizations (shots/vaccines):
- All students entering or attending any New York State school (public, nonpublic, and charter schools) must have received, or have a validated appointment date to receive, all required doses of immunizations for their grade level in order to attend or continue to attend school. The immunization requirements for each grade level are outlined on NYSDOH Immunization Requirements for School Entrance/Attendance Chart. Students who do not have the required immunizations or have a validated appointment to receive required immunization(s), may not attend school.
Prescribed & Over-The-Counter Medications:
- A written healthcare provider order;
*Note: a student is permitted to self-carry only life-saving medications such as an inhaler, or epipen, which still requires a provider’s order indicating that they have the maturity to self-carry. Other medications, including over-the-counter, must be administered through the school.
- Written parent/guardian consent, even for a self-carry student, and;
- Any and all medications must be in their original, labeled, prescription, or over-the-counter bottles/packaging. Any special supplies or equipment to administer the medication must also be provided to the school.
The above requirements for medication administration are required with each new school year. Orders from the previous year cannot be used the following school year.
All medications and supplies must be picked up by the end of the school year or they will be disposed of. Items cannot be stored or housed at the school from school year to school year.
